CarePoint Health (CPH) is an interprofessional, team-based, primary care centre which aims to link with primary care providers in our community to care for the well-being of the population of Mississauga and surrounding areas. Primary care providers within our network have equitable access to team-based care for their patients most in need, regardless of their funding model or practice structure. We provide holistic care inclusive of social determinants of health for wellness, prevention, and management.
CPH supports patients by connecting them to a coordinated, comprehensive team of health care professionals upon whom they can rely on and work with to achieve their personal health goals. We also link into the surrounding programs and services of the community to enable continuity of care across the patient’s journey.
CPH is a hub for which affiliated primary care providers in our community can organize around health system planning, developing community of practice, promoting rapid learning and innovation, and optimizing local infrastructure to achieve greater efficiencies.
Our goal is to achieve the Quadruple Aim of increasing patient satisfaction, improving population health, providing better value for money, and improving provider work life.
